Evaluation of a Social Prescribing Pilot in Shropshire - implementation and impact findings

Introduction: The Shropshire social prescribing service seeks to address real life social complexity and inequalities by offering integrated, holistic support to multifaceted health and social care issues. The service refers people to a link worker who co-produces solutions in a compassionate, non-judgemental manner.
